  • Chronic Disease Co-Care Scheme

    782 participating doctors

    Hong Kong residents aged 45 or above with no known history of diabetes or hypertension can join a subsidised screening and follow-up management pathway through a participating primary care doctor.

  • Colorectal Cancer Screening Programme

    1161 participating doctors

    Hong Kong residents born between 1950 and 1976 can join a subsidised faecal test through an enrolled primary care doctor, with a colonoscopy co-payment capped at HK$1,000 if a referral follows. More than 1,200 primary care doctors are enrolled.

  • Elderly Health Care Voucher Scheme

    3339 participating doctors

    Hong Kong residents aged 65 or above receive HK$2,000 a year in vouchers, usable at registered private primary healthcare providers, with unused amounts carried forward up to an accumulation limit of HK$8,000.

  • Seasonal Influenza Vaccination Subsidy for Outreach Programmes

    331 participating doctors

    A companion outreach arm of the Vaccination Subsidy Scheme, delivering subsidised seasonal influenza vaccination to eligible groups through outreach vaccination teams rather than a clinic visit.

  • Vaccination Subsidy Scheme

    1931 participating doctors

    A government subsidy toward seasonal influenza and pneumococcal vaccination for residents aged 50 or above, children and adolescents, pregnant women, and people with specified chronic conditions, given through participating doctors.
